It has now been determined that he died of hypertension and atherosclerotic cardiovascular disease, the Connecticut Office of the Chief Medical Examiner said.

The disease occurs due to long-term untreated high blood pressure.

Atherosclerosis refers to arteries that become narrowed due to overtime accumulation of fat.

He fell on August 30 while performing at a summer concert.

A video soon surfaced of the rapper cheering for the crowd shirtless, before walking behind the DJ booth.

He continued to try to get the crowd fired up, but took a long pause after saying, “If you came to the party . . .”

He then appeared to lean forward over the cabin saying, “Make some noise.”

At that moment, Freeman’s words trailed off and he suddenly collapsed to the ground.

The medical staff rushed to perform CPR, but unfortunately the rapper could not be saved.

Fans in the crowd started screaming and yelling if Freeman was okay.

Lauren Garrett, Hamden’s mayor, later confirmed in a Facebook post that Freeman had been taken to a nearby hospital.

The following day, fans learned of his death, and many rushed to pay their respects.

The Harlem-born rapper “Be Faithful” is remembered as an icon and dubbed “the undisputed voice of the club,” perhaps best known for the line “Single ladies, make noise!”

His tour manager Birch Michael issued a heartbreaking statement following the shocking death.

It said: “It is with deep sadness and heavy hearts that we share the news of the passing of the legendary and iconic Fatman Scoop.

“The world has lost a shining soul, a beacon of light on stage and in life.

“Fatman Scoop was not only a world-class performer, he was a father, brother, uncle and friend.

“He was the laughter in our lives, a constant source of support, unwavering strength and courage.”

Fatman Scoop had no concerns about his health just before his shocking death, the Mirror reported.

Sharron Elkabas, his representative from booking agency MN2S, said: “He was in good health and in good spirits.

“He sounded in good shape. This news came as a huge and devastating shock, it’s still sinking in.”

Why the name ‘Fatman Scoop’?

The stage name of the hip-hop icon often seemed strange to fans.

But he discovered the origin back in 2015.

“My uncle Jack, God bless the dead, would always take me for ice cream,” Freeman told the Mirror at the time.

“I loved ice cream! My parents would say, ‘Stop feeding that boy ice cream!’ But he said: ‘I don’t care, give him what he wants!’

“He gave me the nickname Scoop and it stuck with me.
