In 2004, he became Congressional District Youth Chairman. He did not contest the election as he left the party after some time. Patil was unhappy with the Congress party’s handling of the protests, according to a leader of the Maratha group Sambhaji Brigade, a 2003 book by James Laine. The controversial book is titled Shivaji: The Hindu King of Islamic India. The book stirred divisions among the Marathas by raising questions about Shivaji’s parents. As a result, important records from the Bandarkar Institute of Oriental Studies that contributed to the study of the book were destroyed. The incident resulted in multiple arrests. To plan demonstrations in support of the Maratha reservation, he formed the Shivba Sanghatana group. In 2009, he joined the movement for employment and education reservation for the Maratha minority. He was supported by six close aides of Jalna district, including Pandurang Tarakh of Sarpanch village, Antarwali Sarati doctor Ramesh Ramesh Tarakh, Pradeep Solunke, teacher at Chhatrapati Sambhajinagar (formerly known as Aurangabad) and three farmers, Shreeram Kurankar, Sanjay Katare and Dhananjay Duphake, are his oldest colleagues. They played an important role in the rallies initiated by Patil. They helped him develop a rough itinerary, including the speeches he would give and arrangements for accommodation and food. In 2013, he embarked on his first protest – a 13-day hunger strike in Shahgad to advocate for the Maratha reservation. He then went on a second hunger strike in front of the Jalna police chief’s office, which lasted for eight or nine days.

In 2016, Marathas staged protests after a Maratha girl was raped and killed by a gang in Kopaldi. Patil also played an important role in these demonstrations. Patil organized the Marathas of Marathwada district and brought them to the state capital for the Maratha Kranti Morcha in Mumbai in 2016-17 ) protests. Patil held rallies across the country in 2021 following the Supreme Court’s decision to abolish Maratha reservation. These included three months of demonstrations in Sasht-Pimpalgaon in Jalna district, where large crowds came out in support of him. In August 2023, Maratha protesters attracted attention during their hunger strike. Chief Minister Shinde convinced him to end the demonstrations by promising to set up a committee to look into the possibility of issuing Maratha certificates to the Kunbi caste. On the fourth day, police attempted to forcibly remove him from the protest. This led to a fight between Patil’s followers and the police. In September 2023, he staged another demonstration, which he ended 16 days later by drinking juice provided by Chief Minister Eknath Shinde.

A group of senior government ministers and retired judges asked him to wait until December 24, 2023. He accepted their offer but also warned them that if they did not meet their demands by a decided date, he would strike again. On December 26, 2023, he announced that he would go to Mumbai for a rally and go on a hunger strike at Azad Maidan until his death. Patil addressed the protest and announced that he would start an indefinite hunger strike on January 20, 2024, at Azad Maidan in Mumbai. He said in a statement,
It was difficult to suppress the Marathas. We won’t be back without booking for the community. I will be walking from my village in Antavalli in Jalna district to Mumbai and will be joined by members of our community. “

On January 27, 2024, Chief Minister Eknath Shinde, in a proposal, said that he would provide Kunbi certificates to 5.7 million Marathas with Kunbi background. He also said that proof would be provided to Maratha family members with Kunbi background by producing affidavits from relatives with Kunbi records. On January 27, 2024, when he canceled his trip to Mumbai, he said the case would be heard by a judge. He and his team later said they wanted to make some changes to the proposal. They also claimed that if their petition is rejected, they will prevent Prime Minister Narendra Modi from addressing a campaign rally in the state for the upcoming national elections. On February 10, 2024, he went on hunger strike again. On the sixth day of the hunger strike, his condition worsened and he required intravenous medication. In an interview, he said of the hunger strike:

I have made it clear to the state government that irrespective of whether its report is positive or negative, it must start issuing Kunbi caste certificates to the Maratha community. If the state government fails to provide reservation to the Maratha community, then in a month I will resume the hunger strike again,” Patil said at a rally, warning the government that he would “not allow” the current incumbents if they reneged on their promise Any minister in the government can move freely in the state.”
