YouTube star Nikocado Avocado reveals another amazing transformation.

The content creator, whose real name is Nicholas Perry, stunned viewers on September 6, 2024 with his 250 lb. weight loss, which he kept secret by posting pre-recorded mukbangs (videos of people eating excessive amounts of food while talking to viewers). Perry’s video titled “Two Steps Ahead” has been viewed 50 million times.

Now he has publicly announced that he is undergoing a facelift to fix his jaw.

“Towards the end of my weight loss journey, my side profile was just horrible,” Perry tells PEOPLE exclusively. “I had no beard at all. There was just this big gobble gobble turkey neck sitting under my face. And so I said, ‘You know what? I have to take care of it.’ ”

Perry, who has 4.6 million subscribers on YouTube, says he’s always struggled with a double chin, even before he lost the weight. “I was talking to my followers about how I didn’t like my dimpled chin. I even have some videos called ‘Say hello to my double chin’. When I gained and lost weight, it was overwhelming.”

In December 2024, he sought the help of dr. Ben Talei, a plastic surgeon at the Beverly Hills Center for Plastic Surgery. Talei tells PEOPLE that Perry “was left with a yard of extra muscle and skin that kind of stretched out over time and then deflated. The only way to get rid of it is surgery,” says Talei.

In the video, Perry shares her pre-surgery jitters. “I don’t want to look like a different person,” he says, adding that he knows he’s in good hands with Talei.

“I’m doing this for myself, I’m doing this for the violin [he’s played for years]I’m doing this for my haters,” he says.

On the day of surgery — as he prepares for Talei’s signature AuraLyft facelift and “weekend neck lift” — Perry is nervous but excited. “I’m very confident it’s going to look good, I’m just worried about the pain,” he says. “I’m just nervous. I hope I don’t pee or poop.”

Because of Perry’s dramatic weight loss, “his relaxation was so great,” Talei recalls. “It has a sort of domino effect — meaning if you lift its neck, it’s going to be so relaxed that it’s going to push in the face and it’s going to be ‘I’ve got all this excess tissue for my face.'”

That’s why Perry also needed an “eyebrow reset,” Talei says. “I had to raise his eyebrow. And that’s what we did to give Nik his jaw back.”

Perry recovered well – “my tooth extraction hurt a lot more,” he says with surprise – and was delighted when the bandages came off. “It’s so strong, it’s so fleshy, it looks like it has an implant,” he says after seeing his new jaw. “I actually cried – twice. Tears of joy.”

As to why he decided to have the surgery now, Perry says he could no longer hide the skin on his neck. Before, “I knew how to hide well by placing the camera at certain angles,” he explains. But when he started planning his Two Steps Ahead tour, he knew that “I would be photographed from all directions and that I would have no way to ‘hide’ my drooping neck.”

“Dr. Ben Talei worked closely with me to plan my facelift … before the start of my tour so that my face would heal properly,” said Perry, who loves the results of his procedures. “It’s beautiful,” Perry says. “I’m happy where I am.”

As for what’s next for Perry, he’ll still be making videos, just not in the same way. The self-proclaimed “theatre kid” tells PEOPLE that he originally “decided to make YouTube my stage, and I just gave up. I got really into trolling and acting, drama, falls.”

He says he’ll continue making mukbangs — “I love that kind of content, I could talk for hours,” he says — but it might look different.

“Instead of five cheeseburgers, I could cook seafood. Instead of a pile of bacon and fried chicken with lots of cheese. I’ll have a nice steak and broccoli.”

And Nicocad’s future plans?

“Nicocado might be the queen of plastic surgery right now,” jokes Perry. “What’s next? A lip, nose, hair transplant? I don’t know!”

In the meantime, Perry will enjoy being “genuinely completely amazed” by his new features.

“I actually have a jaw line now,” he says in the video. “I have what everyone else has, and I’ve always wanted.”
