Etra began his political career in 2003 with the Bharat Rashtra Samithi party. In 2004, he was elected as MLA from Kamalapur Assembly constituency in the Andhra Pradesh Legislative Assembly elections. He serves as the floor leader of TRS. In 2008, he resigned as MLA and was re-elected from Kamalapur constituency. In 2009, he contested the Andhra Pradesh Legislative Assembly elections from the merged Huzurabad and Kamalapur constituencies and was elected as MLA. In 2010, he resigned as MLA and won a by-election. In 2014, he became the finance minister of Telangana. In the same year, he competed with Indian National Congress leader Kesiri Sudarshan Reddy for the Huzurabad constituency and won by a margin of 57,037 votes. Later, in 2018, he was re-elected from Khuzurbad constituency. In 2019, he was appointed as the health minister of Telangana state. In May 2021, Etla was removed from his ministerial post and removed from the state cabinet due to the charges against him. On June 4, 2021, he resigned from BRS and a few days later, on June 12, 2021, he resigned as MLA.

On June 14, 2021, Etela joined the Bharatiya Janata Party in the presence of BJP leaders Dharmendra Pradhan and G. Kishan Reddy. Later, that same year, he contested a by-election and was elected as a provincial councilor. He also resigned as chairman of the Nampally Exhibition Association in 2021.
Rajendar contested the 2023 Telangana Legislative Assembly elections from Huzurabad and Gajwel constituencies; however, he lost to BRS candidates Kaushik Reddy and K. Chandrashekar Rao respectively.

Allegations of land appropriation
In May 2021, there were allegations that Etela Rajender, his followers and wife Jamuna Hatcheries were involved in land encroachment in Achampet and Hakimpet villages in Medak district. The chief minister ordered the setting up of two committees to probe the alleged land encroachment by Rajendal. The first committee, headed by Medak Collector S. Harish, was formed to investigate the allegation that the Jamuna hatchery of the Rajeev family had encroached into designated areas of Medak mandals in Ashantpet and Masaipet. The probe into the Devaryamjal temple land encroachment is being conducted by the second committee, headed by Raghunandan Rao and including IAS officers Prashanth Jeevan Patil, Bharathi Holikeri and Swetha Mohanty. According to reports, the Harish-led committee found that 85 acres of allotted land belonging to 65 allottees had been encroached upon by Rajendhar’s chicken farm.
Rajeed held a press conference claiming he was innocent of any wrongdoing and even dared to be investigated by the Central Bureau of Investigation or a sitting judge into the allegations. After being stripped of his health affairs, Rajeev told media persons that the series of incidents happening around him were meant to defame him. Congress spokesperson Dasoju Sravan said the BJP had called for a high-level committee to probe the allegations against Rajeev.
The chief minister hatched a political conspiracy against Rajeev to “compensate for his failure in containing the epidemic”.
