She dropped out of school in Calicut, moved to Thiruvananthapuram, and completed her education at Kendriya Vidyalaya in Pangode. Parvathy Thiruvothu made her debut in a supporting role in the 2006 Malayalam film Out of Syllabus.
In her second Malayalam film The Notebook (directed by Roshan Andrews), she played one of the three main leads in the film.
In 2007, she played a supporting role in the comedy film Vinodaythra. Parvathy Thiruvothu plays the female lead opposite Puneeth Rajkumar in the Kannada film Milana. The film was a huge hit and ran in a theater in Bangalore for 500 days.
In 2008, she played the role of Maari, a worker at a village fireworks company, in the Tamil film Poo. She revealed in an interview that in order to play the role of Maari, she visited a fireworks factory to grasp the role. She also revealed that she turned down nearly 12 movies for “Poop.”
In 2009, Parvathy Thiruvothu voiced the character herself in the Kannada film Male Barali Manju Irali (2009). In 2013, she starred opposite famous Indian actor Dhanush in the Tamil film Marian. Her role as RJ Sarah in the Malayalam film Bangalore (2014) received wide acclaim from the audience.
In 2015, she starred in the Malayalam romantic biopic Ennu Ninte Moideen, based on the tragic love story of Kanchanamala and Moideen, a couple from northern Kerala. She has received various awards for her performance in films. In 2016, she reprized the role of RJ Sarah in the Tamil remake of the Malayalam film Bangalore, titled Bangalore Naatkal. Her performance in the film “Take Off” (2017) was well received by the audience, and the film was in theaters for more than 125 days. The film was screened at the International Film Festival of India and the Kerala International Film Festival.
Parvathy Thiruvothu made her Bollywood debut in 2017 with the film Qarib Qarib Single opposite Irrfan Khan.
She played an acid attack survivor in the Malayalam film Uyare and won several awards for her performance in the film.
In 2022, she played the role of a pregnant woman in the movie “Wonder Woman” starring Nadia Moidu. The movie will be released on SonyLIV on November 18, 2022.

At the International Film Festival of Kerala (IFFK) 2017, Parvathy Thiruvothu talks about misogynistic dialogues used in Malayalam films and quotes popular Malayalam Take the example of actor Mammootty’s film “Kasaba”. she says,
IMHO, this movie made me very disappointed with the producers because it featured a great actor who delivered misogynistic dialogue. Many people feel and believe that movies reflect life and society. When a superstar utters dialogue like this, people obviously think it’s cool to make sexist remarks. ”
Later, she was severely attacked and received vicious and abusive comments from Mammootty fans on social media.

- Parvathy Thiruvothu is a trained Bharatanatyam dancer.
- Parvathy Thiruvothu was a victim of child abuse, night play and stalking. She revealed this at an event at St Teresa College, saying:
I was a victim of child abuse and eve teasing. I have encountered online predators and stalkers.
- She had always eaten meat, but she became vegetarian after meeting Joan MacArthur (photojournalist) while filming the movie “Ennu Ninte Moideen” in Finland in 2015. She also stopped using dairy products and leather products.
- The actress was attacked and abused on social media for adding the surname “Menon” to her name in support of the caste system. In response, she published a post on Facebook on December 23, 2015, clarifying that she did not have a surname in any official documents and that the surname “Menon” was mistakenly added to her name by a media reporter. She revealed that her birth certificate had only one surname, “Parvathy Thiruvothu Kottuvatta” (Thiruvothu Kottuvatta was the name of her mother’s family). During her schooling, due to some transfer certification errors, she lost this surname and, as a result, her 10th standard certificate, driving license, voter card and passport all bore the name “Parvathy”.
- Parvathy Thiruvothu spoke about Islamophobia in Kerala in an interview; however, she admitted that Islamophobia is also a part of her films and said that she would avoid appearing in films containing Islamophobia Teaching content in movies.
- Parvathy Thiruvothu along with other actresses, directors and technicians from the Malayalam film industry formed Women Film Collective to promote women workers in the Malayalam film industry Welfare. The organization was founded in May 2017 to fight against gender equality.

- In 2020, Parvathy Thiruvothu resigned from the Association of Malayalam Movie Artists (AMMA) in protest against Edavela Babu’s comments to a Misogynistic comments made by an actress.
