Doctors are investigating a recurring problem that forced him to cancel his tour last month.
The former One Direction star was celebrating his first anniversary with Kate when he fell ill at their lakeside property.
He was rushed to the ER by ambulance before being checked into the hospital’s emergency department, where he will remain for several days.
The latest scare comes just weeks after he told fans he had been in hospital with a serious kidney infection, forcing him to cancel his first solo tour.
A source said: “Liam is in a bad way but he is in the best place he can be and the doctors will finally be able to get to the bottom of what is going on.
“Of course he’s upset that his and Kate’s trip to Lake Como fell through, but at least she was there to help him when he got sick.
“The doctors warned him not to expect to be discharged home for at least another six days.
“They want to do all the tests they can to fully understand the problem, but now they suspect there’s been an underlying problem that’s been getting worse for some time.”
Liam, pictured with Kate in Italy the day before he was rushed to hospital, revealed earlier this year that he had been sober for almost six months after drinking heavily in One Direction.
He spent most of the year working on his second album.
But last month he was forced to postpone his South American tour just days before he was due to take the stage.
In a video to fans, he said: “Over the past week I have been in hospital with a serious kidney infection, which is something I would not wish on anyone, and the doctors have ordered that I now need to rest and recover.
“I was beyond excited to come play for you.
“To all of you who bought tickets: I’m so sorry.”
Liam, who has a son Bear with ex Cheryl, also revealed this year that he spent 100 days in a rehab facility in Louisiana.
He told how he often emptied mini-bars on world tours with One Direction before their split in 2015.
But he said he now has “more control over life”.
He told fans: “I just needed to take some time for myself really because I’ve kind of become someone I don’t really recognize anymore.”
He added that the scariest part was turning the phone back on after 100 days.
